(A) Dadasaheb Phalke Award

The Dadasaheb Phalke Award is India's highest award in cinema, presented annually by the Government of India for lifetime contribution to Indian cinema.

The first recipients of the Bharat Ratna were: the former Governor-General of the Union of India C. Rajagopalachari, the former President of the Republic of India Sarvepalli Radhakrishnan; and the Indian physicist C. V. Raman, who were honoured in 1954.

As of my last knowledge update in January 2022, Shyama Prasad Mukherjee has not been awarded the Bharat Ratna. Please note that new awards or recognitions may have been conferred since then, so it's advisable to check the latest sources for the most up-to-date information.

Actor Anupam Kher was conferred the Padma Bhushan by President Pranab Mukherjee in March, 2016.

(D) The most conspicuous bravery or self-sacrifice on land, air, or sea but not in the presence of the enemy.

The Ashoka Chakra is a peacetime military decoration awarded for acts of the most conspicuous bravery or some daring or pre-eminent act of valor or self-sacrifice on land, at sea, or in the air. It is not specific to actions in the presence of the enemy, distinguishing it from some other military decorations.
